Computer-Based Measurement of Polyp Size Getting Closer

Polyp size measurements by endoscopists during colonoscopy are notoriously inaccurate, with both over-measurements and under-measurements. Computerized assessments of polyp size are promising for more accurate measurement. 

In an evaluation of endoscopist classification of phantom polyps of known size in a pig model, endoscopists and a computer model categorized polyps as ≤5 mm or >5 mm. The computer-based sizing system had an average diagnostic accuracy of 85.2%, compared with 59.5% by the endoscopists. For lesions >5 mm, the sizing accuracy was 87.5% for the computer versus 42.3% for the endoscopists.

COMMENT

Computerized sizing, when available, should improve the accuracy of postpolypectomy surveillance recommendations and categorize polyp size accurately for health care systems able to use the resect-and-discard approach to diminutive adenomas. The technology will also be very useful for clinical trials and research studies on colorectal polyps. Hopefully, size measurement programs will become standard equipment in endoscope processors in the near future.
