Endoscopy Unit-Level Measures That Improve ADR: Report Cards and a Second Observer

In a systematic review and meta-analysis, 15 studies evaluating the impact of report cards on adenoma detection rate (ADR) were among the 34 studies that qualified for inclusion. The odds ratio (OR) for the ADR increase after report card implementation was significant at 1.28.

Among other studies and findings, 4 studies evaluated the effect of an additional observer on ADR, which resulted in a significant ADR increase (OR, 1.25), and monitoring withdrawal time (5 studies) did not impact ADR (OR, 1.35; 95% confidence interval, 0.93-1.96).

COMMENT

Endoscopy unit-level steps to improve ADR are important because they can impact all endoscopists. Withdrawal time should still be recorded in every case and monitored as a signal of poor technique. Thus, doctors with a low ADR and short mean withdrawal times need technique remediation.
