Predictors of Endoscopy Procedure Volume During Fellowship: Male Gender and Private (vs Academic) Program

The American Society for Gastrointestinal Endoscopy recommends that gastroenterology fellows complete a minimum of 130 EGDs and 275 colonoscopies before assessing competency.

In a survey of U.S. GI fellowship programs, the response rate was 14% (n=360, 33% female, 48% White, and 88% clinical fellows). Multivariate analysis showed female fellows performed fewer total EGDs and colonoscopies than male fellows (1003 vs 1027; relative risk [RR], 0.94), Northeastern programs had greater procedure volumes than programs in other U.S. regions (RR, 1.31), and academic programs had lower volumes than private programs (RR, 0.54). Also, compared with fellows pursuing general GI as a career, fellows entering motility as their career had lower volumes (RR, 0.66), and fellows entering transplant hepatology had lower volumes (RR, 0.84).

COMMENT

These data suggest some factors that fellowship programs should consider for assessing the appropriateness of endoscopy volumes provided for their fellows and subgroups of fellows.
