New Distal Attachment Combines Hood and Finger Features

Distal attachments that utilize fingers include the Endocuff Vision (Olympus America, Center Valley, Penn, USA) and AmplifEYE (Medivators Inc, Minneapolis, Minn, USA). An alternative approach has been the use of a distal hood, and trials comparing the two approaches have had variable results. A new distal attachment called WingCap (A&A Medical Supply LLC, Seongnam, South Korea) was evaluated in a randomized trial. The WingCap has a clear distal hood projecting over the scope as well as two rows of fingers. 

In a randomized controlled trial involving 537 patients, the adenoma detection rate (ADR) was higher with WingCap than in a control group (37.2% vs 26.6%), as was the adenoma per colonoscopy rate (0.72 vs 0.45). The ADR gain in 297 randomized screening patients was 16.3%. Cecal intubation was successful in 99.3% of the WingCap group. There was no difference in insertion time between the WingCap and control groups.

COMMENT

It will take multiple comparative studies to understand the value of this device, but it seems smart to combine the hood and finger approaches.
